62Q.54 REFERRALS FOR RESIDENTS OF HEALTH CARE FACILITIES.If an enrollee is a resident of a health care facility licensed under chapter 144A or a housing with services establishment
registered under chapter 144D, the enrollee's primary care physician must refer the enrollee to that facility's skilled nursing
unit or that facility's appropriate care setting, provided that the health plan company and the provider can best meet the
patient's needs in that setting, if the following conditions are met:(1) the facility agrees to be reimbursed at that health plan company's contract rate negotiated with similar providers for
the same services and supplies; and(2) the facility meets all guidelines established by the health plan company related to quality of care, utilization, referral
authorization, risk assumption, use of health plan company network, and other criteria applicable to providers under contract
for the same services and supplies.
