     45.0295 Fees. 

    (a) The following fees shall be paid to the commissioner: 

    (1) for a letter of certification of licensure, $20; 

    (2) for a license history, $20; 

    (3) for a duplicate license, $10; 

    (4) for a change of name or address, $10; 

    (5) for a temporary license, $10; 

    (6) for each hour or fraction of one hour of course 
 approval for continuing education sought, $10; and 

    (7) for each continuing education course coordinator 
 approval, $100. 

    (b) All fees paid to the commissioner under this section 
 are nonrefundable, except that an overpayment of a fee shall be 
 returned upon proper application.
