9560.0600 PROVISION FOR MEETING HEALTH NEEDS.

The local agency must meet the health and dental needs of every child by:

A.

assuring that each child has had a health examination in the 12 months before initial placement or has an examination within 30 days after placement;

B.

assuring that a child's ongoing health and dental needs are met and that the child has at least one health examination per year while in placement;

C.

assuring that a Child and Teen Checkup is offered or provided pursuant to Minnesota Statutes, section 256B.0625, subdivision 58, to a child eligible for the medical assistance program;

D.

giving the foster care provider information about the child's immunizations and other pertinent health data with instructions for the record to be kept up to date; and

E.

giving the foster care provider a written authorization for obtaining routine health care for the child with clear instructions as to which health care provider is to treat the child and who is responsible for payment.
