5210.0850 ORDER DENYING VARIANCE.

If the commissioner denies an application for a temporary or a permanent variance, the order denying the variance must include:

A.

the name and address of the person or organization requesting the variance;

B.

the rule, or provisions of the rule, from which the application requested a variance;

C.

the extent and duration of the variance requested; and

D.

a concise statement of the reasons for denying the variance request.
