Penalty assessments for violations of parts 4665.4700 to 4665.9900 are as follows:

part 4665.4700, $300;

parts 4665.4800 and 4665.4900, $200;

part 4665.5000, $250;

part 4665.5100, $100;

E.

that portion of part 4665.5200 that states:

"If smoking is permitted, it shall be permitted only in designated areas.", $200;

F.

that portion of part 4665.5200 that states:

"Bedfast residents may be allowed to smoke only while under the direct supervision of a staff member.", $500;

G.

part 4665.5300, $300;

H.

that portion of part 4665.5400 that states:

"In the event of an emergency or unusual occurrence, such as hospitalization, serious illness, accident, imminent death, or death, the resident's parent or others who maintain a close relationship with the patient shall be notified. The wishes of the resident and a parent about religious matters shall be determined and followed as closely as possible.", $250;
