148.9989 DUTY TO MAINTAIN CURRENT INFORMATION.
All licensees and applicants for licensure must notify the board within 30 days of the occurrence of:
(1) a change of name, address, place of employment, or home or business telephone number; or
(2) a change in any other application information.
