4732.0600 REPORTS OF THEFT OR LOSS OF RADIATION-PRODUCING EQUIPMENT.
Subpart 1.
Telephone reports.
A registrant must report to the commissioner the theft or loss of any radiation-producing equipment immediately after the theft or loss becomes known. The report must be made by telephone or facsimile.
Subp. 2.
Written follow-up reports.
A registrant that is required to make a report by telephone or facsimile must, within 30 days after making the telephone report, make a written report to the commissioner listing the following information:
A.
a description of the registered source of radiation involved, including the manufacturer, model, and serial number;
C.
actions that have been taken, or will be taken, to recover the radiation-producing equipment; and
D.
procedures or measures that have been, or will be, adopted to ensure against a recurrence of the loss or theft of registered equipment.
