4731.4513 BRACHYTHERAPY SOURCE CALIBRATION RECORDS.

A licensee must maintain a record of the calibrations of brachytherapy sources required under part 4731.4455 for three years after the last use of the source. The records must include:

A.

the date of the calibration;

B.

the manufacturer's name, the model number and serial number for the source, and instruments used to calibrate the source;

C.

the source output or activity;

D.

the source positioning accuracy within the applicators; and

E.

the signature of the authorized medical physicist.
