4731.4507 MOBILE MEDICAL SERVICE RECORDS.

A.

A licensee must retain a copy of each letter that permits the use of radioactive material at a client's address of use, according to part 4731.4428, item A, subitem (1). Each letter must clearly delineate the authority and responsibility of the licensee and the client and must be retained for three years after the last provision of service.

B.

A licensee must retain the record of each survey required under part 4731.4428, item A, subitem (4), for three years. The record must include:

(1)

the date of the survey;

(2)

the results of the survey;

(3)

the instrument used to make the survey; and

(4)

the name of the individual who performed the survey.
