4731.4503 DOSAGE RECORDS.

A licensee must maintain a record of dosage determinations required under part 4731.4422 for three years. The record must contain:

A.

the identity of the radiopharmaceutical;

B.

the patient's or human research subject's name or identification number, if one has been assigned;

C.

the prescribed dosage, the determined dosage, or a notation that the total activity is less than 30 microcuries (1.1 MBq);

D.

the date and time of the dosage determination; and

E.

the name of the individual who determined the dosage.
