4720.5540 AQUIFER TEST PLAN CONTENT.

An aquifer test plan must be prepared for the wells in a public water supply system and submitted to the department for approval. The aquifer test plan must contain:

A.

the state identifier;

B.

a map showing the location of the pumping well and the monitoring well or environmental bore hole;

C.

the name and address of the public water supplier;

D.

the name and address of the person preparing the aquifer test plan;

E.

specification of the method used from part 4720.5510, subpart 6;

F.

a description of why the method was selected; and

G.

the existing data and the calculated transmissivity value, if the method selected is one of those specified in part 4710.5510, subpart 6, item B, subitem (1), (2), (5), (6), or (7).
