4617.0075 VENDOR AGREEMENTS.

A duly authorized agent of a retail food store or pharmacy which has been approved to be a vendor shall sign and date a vendor agreement. A separate vendor agreement must be signed for each vendor, except that all vendors with identical ownership may elect to be included in one vendor agreement. A vendor agreement must contain:

A.

the names and addresses of all vendors included in the vendor agreement;

B.

the method by which the agreement must be terminated;

C.

terms that are consistent with this chapter and Code of Federal Regulations, title 7, section 246.12, paragraph (h), as amended;

D.

a statement that disqualification from the WIC program may result in disqualification as a retailer in the Supplemental Nutrition Assistance Program (SNAP) and that such disqualification may not be subject to administrative or judicial review under SNAP; and

E.

an ending date that is not more than three years beyond the beginning date.
