2105.0220 REINSTATEMENT AFTER REVOCATION.

An applicant may apply for relicensure after revocation by meeting the following requirements:

A.

the applicant shall provide documentation of the correction or elimination of the grounds for the revocation;

B.

the applicant shall make application for relicensure, in writing and on forms provided by the board;

C.

the applicant shall pay the fees identified in Minnesota Statutes, section 155A.25;

D.

the salon applicant shall meet the requirements for initial licensure;

E.

the individual applicant shall meet the requirements of part 2105.0140 and the requirements of part 2105.0200, subpart 2, or 2105.0210, subpart 3; and

F.

at least two years have expired from the effective date of the revocation.
