148D.095 LICENSE CERTIFICATE OR CARD.
Subdivision 1.
License wall certificate. (a) The board must issue a new license wall
certificate when the board issues a new license. No fee in addition to the applicable license
fee specified in section
148D.180 is required.
(b) The board must replace a license wall certificate when:
(1) a licensee submits an affidavit to the board that the original license wall certificate was
lost, stolen, or destroyed; and
(2) the licensee submits the license wall certificate fee specified in section
148D.180.
(c) The board must issue a revised license wall certificate when:
(1) a licensee requests a revised license wall certificate pursuant to this section; and
(2) a licensee submits the license wall certificate fee specified in section
148D.180.
(d) The board must issue an additional license wall certificate when:
(1) a licensee submits a written request for a new certificate because the licensee practices in
more than one location; and
(2) the licensee submits the license wall certificate fee specified in section
148D.180.
Subd. 2.
License card. (a) The board must issue a new license card when the board issues
a new license. No fee in addition to the applicable license fee specified in section
148D.180 is
required.
(b) The board must replace a license card when a licensee submits:
(1) an affidavit to the board that the original license card was lost, stolen, or destroyed; and
(2) the license card fee specified in section
148D.180.
(c) The board must issue a revised license card when the licensee submits a written request
for a new license wall certificate because of a new professional or legal name pursuant to section
148D.090, subdivision 2 or 3. No fee in addition to the one specified in subdivision 1, paragraph
(b), is required.
