E Regulating dental benefit plans; requiring dental organizations to make
available to enrollees upon request a description of the terms of coverage;
specifying certain dental benefit plan requirements, requiring the availability
of certain information to participating dentists, group purchasers and enrollees
by dental organizations using utilization profiling as a method of
differentiating provider reimbursement or as a requirement for continued
participation in the provider network; regulating the use of reimbursement
codes, requiring the determination of enrollee benefits on the basis of
individual codes subject to provider and group contracts; prohibiting limits on
discussion of treatment options with patients; specifying certain disclosure
limits, authorizing disclosure by electronic communication (mk, ja)
