Requiring the commissioner of human services to contract with the U special kids
program to provide care coordination for medical assistance (MA) enrollees who
are children with high cost medical conditions; requiring commissioner to create
a referral system; specifying qualifying level of medical expenses; providing
for coordination of all nonmedical case management services to children required
to receive care coordination; providing for statewide availability of care
coordination; requiring the U special kids program to establish and operate an
advance practice nurse telephone triage system; requiring the commissioner to
design, develop and maintain an information management web accessible
application; requiring commissioner to monitor program outcomes for quality
coordination of care and financial savings; appropriating money to commissioner
of human services
