plans; prohibiting the commissioner of human services from renewing certain
state health maintenance organization (HMO) contracts; requiring the
commissioner to contract directly with health care providers to deliver covered
services for current and future eligible medical assistance and MinnesotaCare
enrollees; requiring use of the primary case management model to coordinate
services for certain enrollees
