providing for the use and form of +affirmative consent, prohibiting use as a
required condition of doing business +with financial institutions; providing
for access to and correction of +nonpublic personal information under certain
conditions, authorizing financial institutions to charge a reasonable copying
fee; authorizing attorney+ general remedies for violations; specifying the
