Specifying certain reporting requirements relating to the required +notice
before performance of abortions on minors and certain other women; +requiring
the commissioner of health to prepare a reporting form for use by +physicians
and facilities performing abortions, specifying certain form and +report content
requirements, the physicians or facilities required to annually +submit the
forms to the commissioner and commissioner confidential data +maintenance
requirements; imposing a late fee on certain late reports and +authorizing
certain actions for incomplete reports, classifying the data related +to the
actions; requiring the commissioner to annually issue public reports +providing
certain statistics for the previous calendar year compiled from the +reports,
subjecting the commissioner to certain civil actions or sanctions for+ failure
to comply; authorizing the commissioner to modify certain +requirements by
administrative rule; providing for the award of attorney fees to +plaintiffs;
