A bill for an act
relating to human services; directing the commissioner of human services to seek
federal funding for the development of recuperative health homes.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

Section 1. new text begin DIRECTION TO THE COMMISSIONER; FUNDING FOR
RECUPERATIVE HEALTH HOMES.

new text begin The commissioner of human services shall pursue federal grant funding to develop a
recuperative health home model designed to serve individuals with chronic conditions, as
defined in United States Code, title 42, section 1396w-4(h), who also lack a permanent
place of residence at the time of discharge from an emergency department or hospital. The
commissioner must not seek federal grant funding for or federal approval of any additional
health home service models prior to the development of the recuperative health home model.
