Specifying certain reporting requirements relating to the required notice before performance of abortions on minors and certain other women; requiring the commissioner of health to prepare a reporting form for use by physicians and facilities performing abortions, specifying certain form and report content requirements, the physicians or facilities required to annually submit the forms to the commissioner and commissioner confidential data maintenance requirements; imposing a late fee on certain late reports and authorizing certain actions for incomplete reports, classifying the data related to the actions; requiring the commissioner to annually issue public reports providing certain statistics for the previous calendar year compiled from the reports, subjecting the commissioner to certain civil actions or sanctions for failure to comply; authorizing the commissioner to modify certain requirements by administrative rule; providing for the award of attorney fees to plaintiffs; severability clause
