relating to adoption; requiring the commissioner of
human services to adopt certain rules.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

new text begin By February 1, 2006, the commissioner of human services, in
consultation with the commissioner of health, shall adopt rules:

new text begin (1) to establish required information and procedures for
requests for affidavits of disclosure and affidavits of
nondisclosure; and
new text end

new text begin (2) establishing required information and procedures for
medical history forms related to birth parents of adopted
children.
