Modifying certain provisions regulating tanning facilities; modifying the consumer notice requirement, requiring facility owners or operators to provide adult consumers with a copy of the warning statement before initial exposure and upon contract execution or renewal and requiring minor customers to provide photo identification and to sign the warning statement acknowledging comprehension of the statement and agreeing to wear protective eyewear; restricting use of the facilities by minors under a certain age, requiring physician consent for and parental presence during use; requiring parental written informed consent for use of the facilities by certain older minors
