Modifying or expanding certain immunization notice or statement +requirements for school children and college students; eliminating the requirement+ for notarization of the statement of immunization exemption due to +conscientiously held beliefs; requiring the information provided to parents or +guardians before immunization to include the right to exemption for the beliefs and +requiring recording on medical records of the date of provision of the +information sheet; failure to provide the required information to be grounds for +disciplinary action against the health care provider; requiring hospitals before +discharging newborns to provide the mother or guardian of the newborn with a copy+ of the federal department of health and human services vaccine information +sheets; requiring the commissioner of human services in consultation with the commissioner of health to develop a notice to apprise child care +providers of possible adverse symptoms and side effects of routine child +immunizations, requiring availability of the notice on the department of human +services web site (mk)
