E Relating to insurance; making optional the forms and filings fee quarterly payment method; modifying certain continuing education and reporting requirements for insurance agents and changing the date for nonrenewal of licenses due to failure to complete the education or reporting requirements; authorizing life insurers to increase premium rates for continuation coverage after a certain number of months of continued group coverage; expanding accident and health insurance reconstructive surgery coverage requirements to breast reconstruction following mastectomy, specifying certain coverage requirements and authorizing deductibles, copayments or coinsurance consistent with other benefits under the plan; providing a second six-month medicare supplemental insurance enrollment period for individuals voluntarily disenrolling from medicare part B due to reemployment; extending the time limit for certain persons terminated from health insurance coverage to enroll in the Minnesota comprehensive health insurance plan with a waiver of preexisting conditions limit and modifying MCHA interim assessment authority; modifying certain provisions under the health care administrative simplification act, modifying the definition of uniform dental billing form, providing for the acceptance of electronic and paper claim transactions by group purchasers and modifying certain uniform health care reimbursement documents standards; eliminating the time limit on registration renewals and specifying a form requirement for initial and renewal registration applications by nonlicensed utilization review organizations; providing for external review of MCHA adverse determinations; clarifying the provision providing for use of the security deposit in workers compensation private self insurers insolvency cases; providing for the application of certain licensing requirements for insurance producers, authorizing currently licensed insurance agents to continue to sell, solicit or negotiate insurance under the authority of the agents license; specifying a certain headnote change instruction to the revisor of statutes; repealing the requirement for the commissioner of health after consultation with the commissioner of commerce to periodically adopt certain uniform billing standards (mk, ja)
