69.58 Insuring in unauthorized companies; dues; statement.

The owner of any property situated in any municipality having an organized fire department, or a partly paid or volunteer department, who carries insurance in a company not licensed by this state shall furnish to the commissioner of revenue, on a form prescribed and furnished by the commissioner, a statement, verified by affidavit, showing the description and location of the property, the amount of insurance, in companies not licensed by this state, the owner has effected against loss or damage by fire, the number of the policy, the name and location of the company issuing the policy, and the premiums paid. This statement shall be furnished by those property owners carrying insurance in companies not licensed by this state not more than 30 days after the issuance of the policy of insurance, upon demand of the commissioner, or, if no demand is made, then on or before January 31st, each year. Every such property owner whose duty it is to make this statement who shall willfully make a false statement, or who shall, for 30 days after the demand neglect to render the statement, shall be guilty of a misdemeanor and fined $50, one-half of which fine shall be transmitted to, and disbursed by, the commissioner as other sums collected under the terms of sections 69.58 to 69.61 are disbursed.
