A bill for an act
relating to health care; awarding a grant to train community health care workers
in advanced care directives; appropriating money.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

Section 1. new text begin COMMUNITY HEALTH CARE WORKERS' ADVANCED CARE
DIRECTIVE TRAINING GRANT.

new text begin (a) The commissioner of health shall award a grant to a consortium of stakeholders
to create an initiative to train community health care workers as advanced care facilitators
in order to facilitate respectful, culturally competent conversations on advanced care
directives in communities of color.

new text begin (b) The grant shall be awarded to a consortium that includes Healthforce Minnesota
at Winona State University, a statewide advanced care planning resource organization with
expertise in coordinating community-based strategies, and the Minnesota Community
Health Care Workers Alliance.

new text begin (c) The grant shall be used to:

new text begin (1) train up to 30 community health care workers at a one-day facilitator training
conducted by the Twin Cities Medical Society. Training must focus on critical barriers
related to culture, language, and other factors and build essential skills to be used in the
area of advanced care planning by community health care workers as part of patient
education services;

new text begin (2) conduct a survey of the community health care workers who complete the
training to identify the effectiveness of the training; and
new text end

new text begin (3) prepare a report on the effectiveness of the training, including lessons learned
and recommendations on how to improve the current curriculum for community health
care workers to be shared with interested stakeholders.

Sec. 2. new text begin APPROPRIATION.
new text end

new text begin $14,000 in fiscal year 2017 is appropriated from the general fund to the
commissioner of health for the community health care workers' advanced care directive
training grant program. This is a onetime appropriation.
