A bill for an act
relating to human services; establishing disease management guidelines for
chronic kidney disease; proposing coding for new law in Minnesota Statutes,
chapter 256B.
BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

(a) Any recipient eligible for medical assistance who has been diagnosed with
diabetes or hypertension or who has a family history of kidney disease shall be evaluated
for kidney disease through routine clinical laboratory assessments of kidney function.

(b) Any recipient who has been diagnosed with diabetes or hypertension or who has
a family history of kidney disease and who has received a diagnosis of kidney disease
shall be classified as a chronic kidney patient.

(c) The commissioner shall track recipients who are at risk for chronic kidney
disease or who have been classified as a chronic kidney disease patient to determine
whether these recipients have received appropriate diagnostic testing and treatment
according to generally recognized clinical practice guidelines.

(d) The commissioner shall ensure that participating medical assistance health care
providers are educated and that disease management strategies are implemented in order
to increase the rate of evaluation and treatment for chronic kidney disease according to
accepted practice guidelines, including:

(1) managing risk factors, which prolong kidney function or delay progression
to kidney replacement therapy;

(2) managing risk factors for bone disease and cardiovascular disease associated
with chronic kidney disease;

(3) improving nutritional status of chronic kidney disease patients; and

(4) correcting anemia associated with chronic kidney disease.

(e) As part of the performance reporting and quality improvement system under
section 256B.072, the commissioner shall include in the performance measures used for
the care of diabetes and hypertension screening for reduced kidney function.
