  1.1                          A bill for an act 
  1.2             relating to data collection; requiring forms asking 
  1.3             for information on a completer's racial or ethnic 
  1.4             origin to provide for designation of a multiracial 
  1.5             category; proposing coding for new law in Minnesota 
  1.6             Statutes, chapters 15; and 363. 
  1.7   BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA: 
  1.8      Section 1.  [15.0571] [AGENCY FORMS; DESIGNATION OF RACIAL 
  1.9   ORIGIN.] 
  1.10     An agency governed by section 15.17 that prepares and 
  1.11  distributes a form for completion by an individual that asks or 
  1.12  requires the individual to check or enter information on the 
  1.13  individual's racial or ethnic origin shall include, as an option 
  1.14  to be chosen by the individual, a category designated as 
  1.15  multiracial. 
  1.16     Sec. 2.  [363.0305] [FORMS; DESIGNATION OF RACIAL ORIGIN.] 
  1.17     An employer, employment agency, educational institution, 
  1.18  labor organization, or place of public accommodation that 
  1.19  prepares and distributes a form for completion by an individual 
  1.20  that is related to education, employment, or health care, and 
  1.21  that asks or requires the individual to check or enter 
  1.22  information on the individual's racial or ethnic origin shall 
  1.23  include, as an option to be chosen by the individual, a category 
  1.24  designated as multiracial. 
  1.25     Sec. 3.  [APPLICATION.] 
  1.26     Sections 1 and 2 apply to forms prepared after the 
  2.1   effective date of section 1 and do not require the replacement 
  2.2   of forms on hand and available for use on that date.
