relating to health; appropriating money for advanced care planning on end-of-life
care choices.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

Section 1. new text begin APPROPRIATION FOR ADVANCED CARE PLANNING.

new text begin $500,000 in fiscal year 2018 is appropriated from the general fund to the commissioner
of health to award a grant to a statewide advanced care planning resource organization that
has expertise in convening and coordinating community-based strategies to encourage
individuals, families, caregivers, and health care providers to begin conversations regarding
end-of-life care choices that express an individual's health care values and preferences and
are based on informed health care decisions. $500,000 is added to the base appropriation
beginning in fiscal year 2019 and continuing in each fiscal year thereafter for grants for
advanced care planning.
