A bill for an act
relating to human services; consolidating various home and community-based
service grant programs into the home and community-based services innovation
pool; requiring reports; amending Minnesota Statutes 2018, section 256B.0921;
repealing Minnesota Statutes 2018, section 256B.0658.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

Section 1.

Minnesota Statutes 2018, section 256B.0921, is amended to read:

The commissioner of human services shall

(1) achieving integrated competitive employmentnew text begin for people eligible for publicly funded
home and community-based services

(2) achieving integrated competitive employment for youth new text begin with disabilities new text end under age
25 upon their graduation from school;

(3) deleted text begin livingdeleted text end new text begin supporting and assisting people eligible for publicly funded home and
community-based services to live
new text end in the most integrated settingnew text begin possiblenew text end ; deleted text begin and
deleted text end

(4) new text begin assisting home and community-based waiver service providers projected to receive
at least a ten percent decrease in revenue due to the transition to service rates calculated
under section 256B.4914 in developing innovative service delivery models; and

The commissioner shall seek requests for proposals and deleted text begin shalldeleted text end new text begin maynew text end contract with one or
more entities to provide incentive payments for meeting identified outcomes.

new text begin (a) Recipients of innovation incentive payments shall provide
the commissioner with a report on the activities funded through the innovation initiative in
a format and at a time specified by the commissioner.

new text begin (b) The commissioner shall provide the chairs and ranking minority members of the
legislative committees with jurisdiction over human services with a biennial report
summarizing and synthesizing all the activities and outcomes of the projects funded through
the innovation initiative. The biennial report is due on January 15 of each odd-numbered
year.

256B.0658 HOUSING ACCESS GRANTS.

The commissioner of human services shall award through a competitive process contracts for grants to public and private agencies to support and assist individuals eligible for publicly funded home and community-based services, including state plan home care, to access housing. Grants may be awarded to agencies that may include, but are not limited to, the following supports: assessment to ensure suitability of housing, accompanying an individual to look at housing, filling out applications and rental agreements, meeting with landlords, helping with Section 8 or other program applications, helping to develop a budget, obtaining furniture and household goods, if necessary, and assisting with any problems that may arise with housing.
