  1.1                          A bill for an act 
  1.2             relating to human services; providing funding for a 
  1.3             supportive housing and managed care pilot project; 
  1.4             appropriating money; amending Laws 1999, chapter 245, 
  1.5             article 5, section 24. 
  1.6   BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA: 
  1.7      Section 1.  Laws 1999, chapter 245, article 5, section 24, 
  1.8   is amended to read: 
  1.9      Sec. 24.  [ESTABLISHMENT AND PURPOSE OF THE SUPPORTIVE 
  1.10  HOUSING AND MANAGED CARE PILOT PROJECT.] 
  1.11     Subdivision 1.  [ESTABLISHMENT AND PURPOSE.] If funding is 
  1.12  available, The commissioner of human services may shall 
  1.13  establish a supportive housing and managed care pilot project to 
  1.14  determine whether integrating the delivery of housing, 
  1.15  supportive services, and health care into a single, flexible 
  1.16  program will reduce public expenditures on homeless individuals, 
  1.17  increase their employment rates, and provide a new alternative 
  1.18  to providing services to a hard-to-serve population. 
  1.19     The commissioner of human services may shall create a block 
  1.20  grant program for counties for the purpose of providing rent 
  1.21  subsidies and supportive services to eligible individuals.  
  1.22  Minimum project and application requirements may shall be 
  1.23  developed by the commissioner in cooperation with counties and 
  1.24  their nonprofit partners with the goal to provide the maximum 
  1.25  flexibility in program design.  If any funds are available, the 
  2.1   funds must be coordinated with health care services for eligible 
  2.2   individuals. 
  2.3      Subd. 2.  [COUNTY ELIGIBILITY.] If the commissioner 
  2.4   establishes the pilot project under subdivision 1, A county may 
  2.5   request funding for the purposes of the pilot project if the 
  2.6   county: 
  2.7      (1) agrees to develop, in cooperation with nonprofit 
  2.8   partners, a supportive housing and managed care pilot project 
  2.9   that integrates the delivery of housing, support services, and 
  2.10  health care for eligible individuals or agrees to contract with 
  2.11  an existing integrated program; and 
  2.12     (2) develops a method for evaluating the quality of the 
  2.13  integrated services provided and the amount of any resulting 
  2.14  cost savings to the county and state. 
  2.15     Subd. 3.  [PARTICIPANT ELIGIBILITY.] In order to be 
  2.16  eligible for the pilot project, a county must determine that an 
  2.17  individual: 
  2.18     (1) meets the eligibility requirements of the group 
  2.19  residential housing program under Minnesota Statutes, section 
  2.20  256I.04, subdivision 1; 
  2.21     (2) is a homeless person or a person at risk of 
  2.22  homelessness.  For purposes of this pilot project, "homeless 
  2.23  person" means a person who is living, or at imminent risk of 
  2.24  living, on the street, in a shelter, or is evicted from a 
  2.25  dwelling or discharged from a regional human services center, 
  2.26  community hospital, or residential treatment program, and has no 
  2.27  appropriate housing available and lacks the resources necessary 
  2.28  to access permanent housing as determined by the county 
  2.29  requesting funding under the pilot project; and 
  2.30     (3) is a person with mental illness, a history of substance 
  2.31  abuse, or a person with HIV. 
  2.32     Subd. 4.  [FUNDING.] If the commissioner establishes the 
  2.33  pilot project under subdivision 1, A county may request funding 
  2.34  from the commissioner for a specified number of eligible 
  2.35  participants for the pilot project.  The commissioner shall 
  2.36  review the request for compliance with subdivisions 1 to 3 and 
  3.1   may approve or disapprove the request.  The commissioner shall 
  3.2   transfer funding to be allocated to participating counties as a 
  3.3   block grant and paid on a monthly basis. 
  3.4      Subd. 5.  [REPORT.] If the commissioner establishes the 
  3.5   pilot project under subdivision 1, Participating counties and 
  3.6   the commissioner of human services shall collaborate to prepare 
  3.7   and issue an annual report beginning December 1, 2001, to the 
  3.8   appropriate committee chairs in the senate and house on the use 
  3.9   of state resources, including other funds leveraged for this 
  3.10  initiative, the status of individuals being served in the pilot 
  3.11  project, and the cost-effectiveness of the pilot project.  The 
  3.12  commissioner shall provide data that may be needed to evaluate 
  3.13  the pilot project to counties that request the data. 
  3.14     Subd. 6.  [SUNSET.] The pilot project shall sunset June 30, 
  3.15  2005 2006. 
  3.16     Sec. 2.  [APPROPRIATION.] 
  3.17     $....... is appropriated from the general fund to the 
  3.18  commissioner of human services for the fiscal year ending June 
  3.19  30, 2001, to implement section 1.
