A bill for an act
relating to human services; appropriating money for the collaboration of
community services partners demonstration project.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

Section 1. new text begin COLLABORATION OF COMMUNITY SERVICES PARTNERS
DEMONSTRATION PROJECT.

new text begin (a) $500,000 in fiscal year 2015 is appropriated from the general fund to the
commissioner of human services for a noncompetitive grant to a collaborative of six
disability services providers that provide residential and employment services to fund the
collaboration of community services partners demonstration project to implement:

new text begin (1) an integrated life plan focused on moving people with disabilities into community
living settings with meaningful community employment; and

new text begin (2) an outcome-based wellness program linked to measurable results that supports
healthy living and leads to longer community employment.

new text begin (b) Of the amount appropriated in paragraph (a), $250,000 shall be distributed
equally to the six disability services providers to provide additional staffing hours for
life plan care coordination and wellness program coordination and training for direct
care staff; and $250,000 shall be distributed to the six disability services providers on a
pay-for-performance model, following metrics to be determined by the commissioner of
human services and the collaborative of six disability services providers within 30 days
of the grant award.

new text begin (c) The collaborative of six disability services providers shall consult with the
commissioner of human services to develop an evaluation of the effectiveness of the
demonstration project in increasing the independence of the people with disabilities who
are served by the project. The collaborative shall report the results of the demonstration
project evaluation and make recommendations on expanding the project to the chairs and
ranking minority members of the legislative committees with jurisdiction over health and
human services policy and finance by February 1, 2016.

contrary, the appropriation under paragraph (a) is available until expended.
