  1.1                          A bill for an act
  1.2             relating to marriage; requiring the marriage license 
  1.3             application to indicate that the parties are of legal 
  1.4             age; amending Minnesota Statutes 1994, section 517.08, 
  1.5             subdivision 1a. 
  1.6   BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA: 
  1.7      Section 1.  Minnesota Statutes 1994, section 517.08, 
  1.8   subdivision 1a, is amended to read: 
  1.9      Subd. 1a.  Application for a marriage license shall be made 
  1.10  upon a form provided for the purpose and shall contain the 
  1.11  following information: 
  1.12     the full names of the parties, 
  1.13     their post office addresses and county and state of 
  1.14  residence, 
  1.15     that they are age 18 or older, or if they are minors, their 
  1.16  full ages, 
  1.17     if either party has previously been married, the party's 
  1.18  married name, and the date, place and court in which the 
  1.19  marriage was dissolved or annulled or the date and place of 
  1.20  death of the former spouse, 
  1.21     if either party is a minor, the name and address of the 
  1.22  minor's parents or guardian, 
  1.23     whether the parties are related to each other, and, if so, 
  1.24  their relationship, 
  1.25     the name and date of birth of any child of which both 
  2.1   parties are parents, born before the making of the application, 
  2.2   unless their parental rights and the parent and child 
  2.3   relationship with respect to the child have been terminated, 
  2.4      address of the bride and groom after the marriage to which 
  2.5   the court administrator shall send a certified copy of the 
  2.6   marriage certificate, 
  2.7      and the full names the parties will have after marriage.
