4668.0070 PERSONNEL RECORDS.

Subpart 1.

Scope.

This part applies to all licensees except class C licensees.

Subp. 2.

Personnel records.

The licensee must maintain a record of each employee, of each individual contractor excluded under part 4668.0008, subpart 7, and of other individual contractors. The record must include the following information:

A.

evidence of current professional licensure, registration, or certification, if licensure, registration, or certification is required by this chapter, statute, or other rules;

B.

records of training required by this chapter; and

C.

evidence of licensure under this chapter, if required.

Subp. 3.

Job descriptions.

The licensee shall maintain current job descriptions, including qualifications, responsibilities, and identification of supervisors, if any, for each job classification.

Subp. 4.

Retention of personnel records.

Each personnel record must be retained for at least three years after an employee or contractor ceases to be employed by the licensee.
