A bill for an act
relating to human services; requiring the commissioner of human services to
establish and evaluate a care coordination technology system demonstration project;
requiring a report; appropriating money.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

PROJECT; DIRECTION TO COMMISSIONER.

new text begin (a) For purposes of this section, the following terms have
the meanings given them.

new text begin (b) "Commissioner" means the commissioner of human services.
new text end

new text begin (c) "Provider" means a provider of health care or social and support services.

new text begin (d) "Social and support services" means services designated by the commissioner as
necessary to increase the effectiveness of health care services by addressing individual and
community risk factors and social determinants of health. Social and support services include
but are not limited to those that address the following needs: behavioral health, child care
assistance, economic support, food, employment, and housing.

new text begin (a) By August 1, 2024, the commissioner shall issue
a request for proposals for the design, implementation, and administration of a care
coordination technology system demonstration project that: (1) provides individuals receiving
health care services with improved access to social and support services; and (2) allows
providers and state and county agencies to coordinate care, track service delivery and
outcomes, and identify and address service gaps, for individuals and communities.

new text begin (b) The demonstration project must operate in at least two counties within the
seven-county metropolitan area for the period of January 1, 2025, through June 30, 2026.

new text begin (c) To be eligible to respond to the request for proposals, an entity must demonstrate
that it has worked successfully with other states to develop technology systems that meet
some or all of the criteria specified in subdivision 3.

new text begin (d) In developing the request for proposals, the commissioner shall consult with the
commissioner of health, health care and social and support services providers, and county
human service and public health agencies.

new text begin The care coordination technology system
must:
new text end

new text begin (1) allow providers and state and county agencies to screen individuals for health care
and social and support services needs, identify needed services through embedded screening
and other tools, and electronically refer individuals to other providers using a closed-loop
referral process;
new text end

new text begin (2) allow providers and state and county agencies to track the health care and social and
support services provided to an individual and the efficacy of those services;
new text end

new text begin (3) allow a provider to seamlessly communicate in real time with other providers serving
the same individual and securely share information about the individual, subject to the
consent of the individual;

new text begin (4) aggregate and visualize data related to the delivery of health care and social and
support services to improve care coordination for individuals and communities at risk of
poor health outcomes due to a lack of appropriate social and support services;

new text begin (5) deliver information to providers using a software as a service (SaaS) application that
is accompanied by ongoing training and technical support for providers and state and county
agency staff;

new text begin (6) have the capability to be integrated with existing methods of health care delivery
and coordination, including but not limited to care delivery through managed care
organizations, integrated health partnerships, and health care homes;

new text begin (7) have the capability to be integrated with existing provider, state agency, and county
systems for screening, care coordination and case management, and data management; and

new text begin (8) provide for the consent of the individual and require compliance with federal and
state data privacy provisions.

new text begin Evaluation. new text end

new text begin (a) The commissioner shall evaluate the extent to which the
demonstration project has achieved the requirements of this section. The commissioner
shall report the evaluation to the chairs and ranking minority members of the legislative
committees with jurisdiction over health and human services policy and finance by December
15, 2026. The report must include recommendations on whether the demonstration project
should be expanded to include all areas of the state.

of health, health care and social and support services providers, and county human services
and public health agencies.

Sec. 2. new text begin APPROPRIATION; CARE COORDINATION TECHNOLOGY SYSTEM
DEMONSTRATION PROJECT.

new text begin $....... in fiscal year 2025 is appropriated from the general fund to the commissioner of
human services to establish and evaluate the care coordination technology system
demonstration project. This appropriation is available until December 31, 2026.
