A bill for an act
relating to human services; establishing an outcome-based personal care
assistance pilot project; establishing project requirements; requiring a report;
appropriating money.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

Section 1. new text begin PROGRAM ESTABLISHED.

new text begin (a) The commissioner of human services shall provide a grant to A Chance to Grow
Inc. to establish an outcome-based personal care assistance pilot project in Ramsey
County. A Chance to Grow shall select one or more families to participate in the project.

new text begin (b) To be eligible for the project, a family must have a demonstrated record of
providing high-quality personal care assistance to a disabled child of any age for at least
three years through:

new text begin (1) active parental participation in and supervision of service provided by the
person's personal care assistant;
new text end

new text begin (2) mentoring to improve personal care assistance skills;
new text end

new text begin (3) use of other therapeutic services to complement personal care assistance services;
new text end

new text begin (4) avoidance or substantial reduction of hospitalization of the person; and
new text end

new text begin (5) maintenance of the person's functioning level.

new text begin (c) Selected families shall use the grant money for one or more of the following
purposes:

new text begin (1) to increase personal care assistant hours by up to 20 percent;
new text end

new text begin (2) to increase the personal care assistant hourly rate by up to $3, based on education
and experience;
new text end

new text begin (3) to provide personal care assistance services by two personal care assistants
at the same time when the arrangement would allow therapy to be performed more
expeditiously; or
new text end

new text begin (4) to reimburse other needed services for which funding is not available including,
but not limited to, physical, occupational, and speech therapy.

new text begin (d) The goal of the project must be to demonstrate that the activities supported
under paragraph (c) will:

new text begin (1) result in reduction of future supplementary support;
new text end

new text begin (2) prevent deformities; mobility loss; new, recurrent, or chronic skin breakdown;
wounds; respiratory conditions and illnesses; and infections;
new text end

new text begin (3) maintain the individual's current level of range of motion;
new text end

new text begin (4) increase independence in activities of daily living, such as grooming, dressing,
and bathing, resulting in a decreased future need for supplementary support;

new text begin (5) result in improvements in the person's medical condition; and
new text end

new text begin (6) increase the ability of individuals to communicate their wants and needs
effectively in their surrounding environment.

Sec. 2. new text begin REPORT.
new text end

new text begin The commissioner of human services shall report to the chairs of the senate and
house of representatives finance divisions with authority over the personal care assistance
program by January 15, 2015, on the pilot project, including its impact on the health,
community involvement, and functioning level of participants. The report must include
recommendations on whether the pilot project should be extended, expanded, or modified,
and on the anticipated costs and savings of the recommendations.

Sec. 3. new text begin APPROPRIATION.

new text begin $50,000 is appropriated from the general fund to the commissioner of human
services for each year of the biennium beginning July 1, 2011, for the purposes of section
1. Base level funding for this appropriation shall be $50,000 for each year of the biennium
beginning July 1, 2013. Base level funding shall be zero for the biennium beginning
July 1, 2015.
