A bill for an act
relating to health; appropriating money for grants to support healthy children,
families, and communities; requiring a report.
BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF MINNESOTA:

(a) The commissioner of health shall award grants to support the health of children,
families, and communities through promotion of safe, stable, nurturing relationships and
environments, and social, cultural, and economic well-being.

(b) The goals of grants shall be to build the capacity of community institutions to:

(1) identify and expand opportunities to strengthen population health and well-being
outcomes, with a focus on creating conditions to support healthy children, youth, families,
and communities; and

(2) focus on community level policy, systems, and environmental changes that
support safe, stable, nurturing relationships and environments, and social, cultural, and
economic well-being.

(c) To be eligible to receive a grant under this section, an applicant may be a local or
tribal government, nonprofit entity, or community agency, and must:

(1) demonstrate the ability to convene, coordinate across agencies and systems, and
implement evidence-based and practice-based strategies at the local level; and

(2) submit a plan that includes a community leadership team, community partners,
and a community action plan that includes an assessment of areas of strengths and needs,
proposed action strategies, technical assistance needs, and a staffing plan.

(d) When making grants under this section, preference shall be given to applicants
that serve communities where the percentage of households with children under five years
of age living below the poverty level exceeds 25 percent of the community population, and
the percentage of the nonwhite population is 40 percent or greater of community population.

(e) The commissioner shall provide or coordinate content and technical expertise
and training on strategies in the following areas:

(1) data collection and analysis;
new text end
new text begin
(2) community engagement;
new text end
new text begin
(3) social determinants of health and health equity;
new text end
new text begin
(4) policy, systems, and environmental change;
new text end
new text begin
(5) child development in the context of relationship and experience; and

new text begin
(6) community asset building and risk behavior reduction.

(f) The commissioner in partnership with grantees must create parameters for
evaluation and reporting.

(g) The commissioner shall submit a report under this section to the senate and house
of representatives committees with jurisdiction over health, human services, and education
by February 1, 2016, and by February 1, 2017. The report shall include the name of the
grantees, the purposes for which the grant funds were used, the outcome parameters for
the grant, and the results of the evaluation for each grant awarded.

$1,000,000 is appropriated in fiscal year 2016 from the general fund to the
commissioner of health for the partnerships for healthy futures grant program. This is
a onetime appropriation.
