employer subsidized insurance program to subsidize premiums for employer
subsidized health coverage for certain low income families with children between
certain ages, requiring federal approval; specifying eligibility requirements
and requiring the commissioner to determine cost effectiveness by comparing the
cost otherwise incurred to provide medical assistance (MA); specifying coverage
requirements; providing for reimbursement to the family and to the health care
provider; requiring premiums paid by the families to be determined according to
a sliding fee scale to be established by the commissioner; providing for
